PARTICIPATION FORMS
The TSSAA, CPA’s athletic conference, requires each high school student to submit a physical and a parent consent form prior to participation in any TSSAA sport. A physical is valid for 12 months and can fulfill the TSSAA requirement for every TSSAA affiliated sport throughout the school year. A parent consent form is sport specific and therefore required to be submitted for each sport a student participates in.
CPA requires a transportation consent form be on file for each student that participates in a high school sport. This form is used by each coach to ensure the safety of your student as well as follow the wishes of the parent in regards to who the student can ride with to a sporting event.
The required forms are below and can be downloaded and printed at your convenience. The forms are also available in the athletic department if you would like to stop by and pick one up. If you find you have questions, please contact the athletic department at 301-3573.
